Observation of Fetal Outcome and Post-Partum Complications among Patients with Placenta Previa After Cesarean Section

Abstract

Background: Placenta previa and its associated complications present significant risks to both maternal and fetal health, especially in cases following a cesarean section. Despite the high prevalence of this condition, there is a lack of comprehensive studies focusing on fetal outcomes and post-partum complications in a Bangladeshi context. Material & Methods: This cross-sectional, hospital-based observational study was conducted at the Department of Obstetrics and Gynecology, Dhaka Medical College Hospital, Dhaka, Bangladesh, over a period of six months. A total of 50 pregnant women diagnosed with placenta previa and with a history of previous cesarean section were included. Data were collected using a structured questionnaire and analyzed using SPSS software. Results: The study found that 86% of the participants were diagnosed with placenta previa, and 14% had placenta accreta. The most common presenting complication at admission was P/V bleeding (88%). A majority of the participants (90%) did not experience any postpartum complications. The study also revealed that most participants had a hospital stay of ≤7 days (84%) and favorable fetal outcomes, with 88% resulting in live births. Conclusion: The study provides valuable insights into the maternal and fetal outcomes among patients with placenta previa undergoing cesarean sections in Bangladesh. The findings underscore the need for targeted interventions and ongoing research to improve both maternal and fetal outcomes. Early diagnosis and understanding of presenting complications like P/V bleeding are crucial for effective clinical management.

Lifestyle of the Patients Suffering from Amoebic Liver Abscess: A Hospital Based Study

Abstract

Background: The liver is the organ most subject to the development of abscess. Liver abscess made up to 13% of the total number of abscesses or 48% of all visceral abscesses. 10% of the world’s population is chronically infected with Entamoeba histolytica, with an incidence exceeding 30% in tropical and subtropical regions. The aim of this study was to describe the lifestyle of the patients suffering from amoebic liver abscess. Material & Methods: This was a cross-sectional type of descriptive study and conducted in the Rajshahi Medical College Hospital during the period of January 2010 to December 2010. In our study, we included 73 patients between the age of 14 years to 55 years both male and female. Clinically detected and confirmed cases of amoebic liver abscess. Socioeconomic status was ascertained by monthly income. Patients who had monthly income less than 3,000 taka were considered as the low income group. Monthly income between 3,000- 20,000 taka was the middle income group and monthly income above 20,000 taka was considered as the high income group. Results: Majority (39.7%) of our patients were aged 25-34 years old & most of our study patients were male (91.7%). The mean age was 35.3±8.71 years with a male female ratio 13.6:1. Most of our study patients (56.2%) were from rural area. Majority (57.5%) patients had education at primary level. Conclusion: From this study we can conclude that amoebic liver abscess is commonly seen in middle-aged alcoholics with male preponderance. It is seen most commonly in low socio economic group with poor sanitation.

Impact of Prolonged Labour on the Baby in Terms of Morbidity and Mortality

Abstract

Background: The negativity associated with a complicated labour such as prolonged labour can lead to a struggle to become a healthy mother and give birth of a healthy child. Delayed delivery may result in various negative consequences, among which still birth is one. The aim of the study is to find out the impact of prolonged labour on neonates. Material & Methods: This prospective study was conducted in Department of Obstetrics and Gynaecology in Sir Salimullah Medical College, Mitford Hospital, Dhaka, Bangladesh during July 2011 to December 2011. 100 pregnant patients with prolonged labour (more than 18 hrs) were selected for this study. Results: 32% patients with multigravida and 68% patients with primigravida got admitted in the hospital. 61% of the total patients had labour pain 18-24 hours. Majority of the patients faced difficulties with dehydration and temperature, 64% and 69% respectively. 77 of the total patients went through caesarean section. Out of 100 expected newborns, 73 was live birth and 21 faced neonatal asphyxia. Conclusion: According to the observation, prolonged delivery can result to some unexpected outcome during childbirth which can be avoided by taking some essential steps earlier.

Abstract

Background: Laparotomy: Abdominal surgery for diagnosis (exploratory) or treatment (therapeutic), especially important for conditions like cancer staging and endometriosis. Clinical assessment crucial, especially in resource-limited settings. The aim of this study was to evaluate the histopathological diagnosis in informing laparotomy surgery indications at a tertiary care hospital. Material & Methods: This was a cross-sectional study and was conducted in the Department of Obstetrics and Gynaechology of Sir Salimullah Medical College Mitford Hospital, Dhaka, Bangladesh during the period from May 2008 to April 2009. Results: In total 110 patients from female patient who were histopathologically diagnosed of oral cancer were included in the study. In our study we found mean age of patients was 37.55±15.58 years. We found all patients had abdominal pain and the majority of the patients had 55% ovarian tumor. We found the majority 30% had benign ovarian tumor and 22% had malignant ovarian tumor and histopathological findings. Out of all patients 31.0% had malignant ovarian tumor, 21.0% had benign ovarian tumor respectively. Conclusion: Laparotomy is a vital surgical procedure for both diagnosis and treatment of abdominal conditions. Properly chosen and performed, it can be life-saving and greatly enhance patients’ quality of life.

Efficacy Of Magnesium Supplementations on Metabolic Profiles of Women with PCOS An Observational Study

Abstract

Background: Polycystic ovary syndrome (PCOS) is affecting women worldwide. This women face difficulties and various outcomes due to consumption of different food and minerals. This study aims to find the impact of magnesium supplements on metabolic profile of women with PCOS. The aim of this study was to observe the effect of magnesium supplementation on metabolic profiles in women with PCOS. Material & Methods: In this clinical trial, 120 female patients with PCOS aged 20-46 years were selected. After definition for body mass index (BMI), age, and types of medications, participants were randomly assigned to consume magnesium supplements (containing 250 mg magnesium oxide). To survey biochemical indicators, a venous blood test was taken after an overnight fasting. The mean age of study participants was 26.8 years. Results: In this study, magnesium supplementation for 8 weeks among women with PCOS had favorable effects on BMI. In addition, the supplementation lead to preventing the increase in waist circumference in intervention group. No significant effects on glycemic variables and lipid profile were seen following the magnesium supplementation. A significant decrease was found in serum testosterone levels. A significant increase in serum dehydroepiandrogens (DHEA) (136.32 vs. 172.37 intervention, 102.74 vs. 120.15 placebo, P = 0.01) was seen in two groups. Magnesium supplementation had no significant effects on FSH, 17OH-progesteron, and free androgen index (FAI) levels. Conclusion: According to the observers, evidences of this study indicate that magnesium supplementation did not influence serum lipid profiles and glycemic indicators among women with PCOS. One the other hand, magnesium supplementation resulted in reduced BMI and testosterone levels as well as increased DHEA concentrations in women with poly cystic ovarian syndrome.

Prevalence And Outcomes of Elective C-Section in A Tertiary Care Hospital

Abstract

Background: Cesarean-section (CS) delivery (Cd) and vaginal delivery (VD), both are related with notable measurable short-and long‑term maternal and neonatal difficulties and advantages. Though normal delivery is expected but some conditions may provoke C-section. The present cross sectional study was conducted on a preliminary basis to evaluate the prevalence of CS delivery in a tertiary care hospital. It also studies the outcome of elective C-section. Material & Methods: This cross sectional study was conducted in the Department of Obstetrics and Gynaecology in Bangabandhu Sheikh Mujib Medical College, Bangladesh. The study duration was 6 months, from January 2010 to July 2010. A total of 100 patients undergone elective C-section were selected for this study. Ethical clearance was taken from the medical authority as well as written consent for this research was taken from the patients and guardians. Results: 42 people of the study patients were of 25-29 years old and majority of them were housewife (73%). Singleton (88%) cases were major followed by 79% of patients who had the delivery after 37 weeks. Mainly headache (31%) and UTI (31%) were the common complications after the delivery. Prematurity was common complication among the neonates after CD (21%). Conclusion: The present study acknowledged that C-section delivery system ensures feto-maternal safety. In some cases, C-section is the only solution to save the life of the mother and the neonate. However, the study also highlighted some probable indications that might engender caesarian delivery.

Challenges And Issues Faced by Junior Doctors During Covid-19 Pandemic in A Tertiary Care Institution: A Mixed Method Approach

Abstract

Background: The world was not ready in terms of infrastructure & manpower to face COVID-19 when it was declared pandemic in March 2020. Health-care workers had to deal with the pressures of changing protocols for treatment as well as the physical & psychological stress. This study was done to assess the challenges faced by junior doctors while performing COVID-19 related duties. Material & Methods: A cross-sectional mixed-method approach study was conducted among 163 junior doctors comprising of Interns, Junior Residents and Post graduate trainees working in a government tertiary care institution in Manipur, India. For qualitative component fifteen (15) In-depth interviews were taken. A structured questionnaire for quantitative and interview guides for qualitative component were used. Descriptive statistics were generated and qualitative data were analyzed thematically. Ethical approval was obtained from the Institutional Ethics committee. Results: Mean (SD) age of the respondents was 27.44±3.2 years with males constituting 46.6%. More than half of the respondents (87, 53.3%) were not satisfied with work environment. Most respondents (121, 74.2%) were unsatisfied with drugs & consumables supply. From the qualitative component, effect on academics due to pandemic, fear of stigma and discrimination, resource constraints, conflicting situations were some of the issues identified.  Conclusion: Most of the junior doctors who had done Covid-19 related duties faced various difficulties and psychological issues. The need to support physical and mental health of our frontline healthcare workers is to be emphasized.

Assessment Of Oral Health Status and Treatment Needs in Geriatric Day Care Centers in New Delhi

Abstract

Background: Poor oral health plagues the elderly all over the world. Many do not recognize the severity of tooth loss, dental caries, and periodontal disease that affects them and do not seek treatment. The objective is to assess the oral health status and treatment needs amongst elderly population visiting the old age homes in New Delhi. Material & Methods: A cross‑sectional study was conducted among seven purposively selected geriatric day care centers in New Delhi, and a convenience sample of 518 elderly patients was obtained. The WHO Oral Health Assessment Form 1997 was used to record the clinical findings. Statistical analysis was done using Statistical Package for the Social Sciences (SPSS) software version 28, and descriptive results were obtained. Results: 428 males (82.6%) and 90 females (17.3%) were examined. The older adults presented high tooth loss with an average of 7 missing teeth, and consequently a high prosthetic need with poor denture hygiene. Most of the older adults were categorized as having “fair” oral hygiene but almost all respondents presented some degree of periodontal disease. Conclusion: The current study highlighted that majority of the patients reported to be satisfied with their oral health status, but most had periodontal problems followed by increased need of prosthesis.

Evaluate the Effectiveness of DENTE91 DB toothpaste: Results from In Vitro Studies

Abstract

Background: Dental problems are common in patients with diabetes. Patients who have poor glycemic control are more likely to have the oral problems such as salivary gland dysfunction, burning mouth feeling, periodontal disease, sluggish wound healing, and increased susceptibility to infections. DENTE91 DB toothpaste is developed to offer beneficial effects in patients with diabetes. Material & Methods: In vitro studies were conducted to evaluate the effects of DENTE91 DB toothpaste on salivary glucose levels and prevention of dryness of mouth. Also, the antimicrobial activity, effects on good oral microorganisms, remineralization, healing, hypersensitivity, and protection against dental caries/tooth decay were evaluated. The major components of DENTE19 DB toothpaste include sorbitol, silica, xylitol, nano hydroxyapatite (nano hydroxyapatite dispersion), cocamidopropyl betaine, sodium methyl cocoyl taurate, xanthane, and PEGB. Results: The percent improvement in reduction of glucose, based on the glucose oxidase enzyme’s enzymatic activity on the glucose, was greater (average 11%) with DENTE91 DB toothpaste versus control. Additionally, a 29% decrease in dryness of mouth was seen when using it. Significant antibacterial action was also shown by DENTE91 DB toothpaste against gram-negative, gram-positive bacteria, and fungi. Additional positive effects included: i) high survival rates of good oral microorganism; ii) significantly reduced lesions of teeth surface and enhanced remineralization process; iii) significantly increased NIH/3T3 cell proliferation and healing activity on dental cavities; iv) greater control of hypersensitivity through significant coverage of the lesions on dentine discs; and v) less mineral loss compared to controls. Conclusion: DENTE19 DB toothpaste demonstrated a significant decrease in salivary glucose levels and prevention of dryness of mouth, and hence, can be used in patients with diabetes. Additionally, it exerted beneficial effects in terms of antimicrobial activity, survival of good oral microorganisms, accelerated remineralization and healing, decreased hypersensitivity, and protection against dental caries/tooth decay.

The Emerging role of Hyaluronic Acid in Dental Implant Procedures

Abstract

Hyaluronic acid (HA) is a naturally occurring biopolymer (mucopolysaccharide), which has important biological functions and is produced in the human body. It is widely employed in drug delivery applications due to its excellent physiochemical properties. HA is an essential glycosaminoglycan found in the extracellular matrix of all body tissues. Due to the scientific advances made in recent years, numerous uses of HA in dentistry have been established, especially in post-surgical healing, periodontitis and in implant surgeries. Owing to its hygroscopic, viscoelastic, antioxidant, and cell regeneration properties, HA favors regeneration of not only the soft tissues like gingiva and oral mucosa but also aids in osseointegration process of implants. Additionally, it shows anti-inflammatory action by reducing inflammation, accelerates the healing process, and shows bacteriostatic properties, thus reducing the risk of infection after surgery. By improving the connection between implant and bone, HA accelerates osteogenic cell differentiation, which would have a favorable effect on the osseointegration of dental implants. Results from various clinical trials have suggested the role of HA in combination with fillers or as a monotherapy in promoting bone healing and the formation of new bone. Overall, the findings from available literature show that HA favors the healing process and plays an essential role in the management of symptoms of post-implant surgery.

A Comparative Evaluation of Different Zirconia CAD/CAM Milled Three-Unit FPDS with Two Variable Connector Design -An In Vitro Study

Abstract

Background: This study was done to compare and evaluate the effect of different connector designs in 3- unit FPD fabricated from different zirconia materials using CAD/CAM on the fracture resistance. Total forty 3-unit zirconia frameworks with two different connector designs were designed and milled using two commercially available zirconia blanks and were divided into four groups. Material & Methods: Each group consisted of 10 samples. The FPD frameworks were cemented on metal dies using dual cure resin cement. The specimens were loaded axially at the centre of the pontic at a speed of 1.0 mm/min in a universal testing machine. Loading was continued to the point of fracture and failure loads. Results: The propagation of crack pathways was oblique from gingival embrasure to occlusal direction through the connector and pontic in some of the samples. Post-hoc Bonferroni test was used for multiple comparisons after the application of the ANOVA test for comparison within the group. For group 1 (3616.79 ± 78.71), Group 2 (3545.83 ±74.16), group 3 (5812.70± 72.34), Group 4 (5655.84± 83.51) N. Fracture resistance was found to be highest in group 3 followed by group 4, group 1 and lowest in group 2. Conclusion: By increasing the dimension of connector from 9mm2 to 12mm2 the fracture resistance of zirconia increased by 62.22% for group 1 and 3, 62.69% for group 2 and group 4. The fracture resistance was highest for AIDITE zirconia with 12mm2 connector design.
